- Complete forms (1) week prior to your scheduled visit
- Allow you to customize your reminder settings
Setting up your patient portal is quick and simple. Some important things to remember when registering:

- You must use the email address you received this correspondence at.
- You must use your legal First and Last name that you have on file with our office.
